- Access: Members (anaesthetists) will be able to access the site with the user name and password. It is the members’ responsibility to always maintain the confidentiality of this information. Patients will be able to access the questionnaire with the doctor ID code provided by their anaesthetists. Each patient will only be able to submit the questionnaire once, and subsequent access to the questionnaire shall be restricted.
- Privacy: Anaesthetic Care provides an online solution for the mandatory pre-operative patient assessment required by anaesthetists. Anaesthetic Care collects information such as, but is not limited to, your name, phone number, birth date, email address, weight, height, surgical procedure details, health fund membership details, and other medical and lifestyle related information. It is imperative that you answer the questionnaire to the best of your ability and honestly in order to minimise the risks associated with anaesthesia and improve your safety. The answers you submit is directly sent to your anaesthetist via their unique doctor ID code, who will then use the collected information to best plan for your anaesthesia needs. Anaesthetic Care respects your full privacy, and any information collected will not be disseminated, published, broadcast, or communicated by Anaesthetic Care directly to any third party and other anaesthetists, either verbally or in written form. Anaesthetic Care retains such collected information for further reviewing by your anaesthetist until such time the anaesthetist permanently deletes the submission, or 1 month after the date of operation at which point the submissions are automatically and permanently deleted. Anaesthetic Care does not take responsibility for information disseminated by your anaesthetist to other personnel in the medical industry that is involved in supporting your procedure.
